By proceeding with payment, I authorize the Illinois State Treasurer’s Office to conduct a background screening using a secure system provided by Boulevard & Co. I understand that Boulevard & Co provides system access and payment processing only and does not make employment or screening decisions.

I acknowledge that the background screening system processing fee is non-refundable once payment is submitted.
